Ranking and Classifying Medical Diagnoses

As you add each medical diagnosis to a resident's chart, you can designate the rank and classification. You can also select the Rank Diagnoses option in the resident's Med Diag tab to verify and update the rank and classification, as needed.

When you rank the medical diagnoses, you can select the order the medical diagnoses appear on the Medical Diagnosis list, as well as on the UB-04. For example, Principal Diagnosis and Other or Primary, Secondary, and so on.

Classifying the medical diagnoses allows you to track the medical diagnoses based on the origin of the onset. For example, Admission, During Stay, and so on.

The medical diagnosis ranking must be re-evaluated as the resident's condition changes. For example, the resident is diagnosed with a Urinary Tract Infection during their stay.

A diagnosis with a classification of History flows to the MDS 3.0 assessment and the UB-04, unless there is a resolved date.

Note

After you save a medical diagnosis, the Medical Diagnosis Ranking screen opens, allowing you to verify the medical diagnosis ranking and classification.

  1. Click Rank Diagnoses.

  • Classification - Select option. Allows you to track the medical diagnoses based on the origin of the onset.

    • Admitting Dx - The primary diagnosis that can also be the Principal Diagnosis and Admission Diagnosis on the UB-04.

    • Admission - An active diagnosis at the time of admission.

    • Discharge - A diagnosis at the time of a discharge or the reason for the discharge.

    • During Stay - A diagnosis during the resident's stay.

    • History - A diagnosis previous to admission.
