Documenting Allergies

You can document allergies for residents to ensure the safe administration of medications, consumption of foods, and exposure to items in the environment. Allergen and reaction libraries provide a structured format for you to document and monitor resident allergies to ensure that adverse allergy reactions are not overlooked.  After entering allergies for a resident, they appear in the resident, Point of Care, and eMAR headers in red font for easy visibility.

Tip

It is recommended you enter an allergy from the Drug category, using either Medispan search libraries. If enabled, allergy checks are completed on medication orders for Drug category allergies only.

Procedure

  1. Clinical > Resident > Allergy.  
  2. Click New.

  1. Enter allergy as Drug. If not found, select another category.
  2. Complete the fields as required.
  3. Click Save.

Options

Below is a list of the options available to you.

  • Status - Select option. When selecting Prior History, the allergy does not appear in the resident header, allergy listing, or on some reports.
  • Date - Enter or select option.
  • Resolved Date - Enter date.
  • Category - Select one of the following options. 
    • Drug - Use the Medispan - Allergen or Medispan - Therapeutic Class libraries to generate an allergy alert for medication orders entered. Custom searches your Custom Medication Library.
    • Food - Includes items such as Watermelon, Chocolate, or caffeine.
    • Environmental - Includes items such as dogs, dust, or adhesive tape.
    • Substance - Includes items such as vaccines, waxes, or x-ray contrast agent.
    • Other - Type an allergy if not found in above categories.
  • Allergen - Type information.
  • Allergy Type - Select option.
  • Severity - Select option.
  • Reaction Type - Select option. If Anaphylaxis it selected, an icon appears in the allergy listing.
  • Reaction Sub Type - Select option. Specific Reaction Types trigger this additional field.
  • Reaction Note - Type information. If entered, a note icon appears. Hover to see notes in allergy listing.

Hints and tips

  • Use the Medispan - Therapeutic Class library when you know the resident is allergic to an entire class of medications. Use the Medispan - Allergen library if the resident is allergic to medications containing a certain ingredient. 
  • You can view and add allergies and access the Allergy Report from the Active Allergies pane on the Resident Dash.
  • When updating an allergy, you cannot modify the Status, Date, Category, Allergen, or Allergy Type. To make these changes, you need to strike out and create new. 
  • The H link in the allergy listing or the Allergy Audit Report under the Allergy Tab provides a history of changes for specific allergens.
