Clinical Reports & Descriptions

This Quick Guide lists the common Clinical Reports in PointClickCare (PCC). Reports are available by clicking on the Reports under Clinical /Admin. In some cases there are shortcuts from other screens. Most reports may be set up for specific time frames, for individual resident or all residents, by unit or entire center, and sorted a variety of ways. 

 

Report

Description

Daily Census Report

Provides a listing of beds, with resident, care level, type of bed, and primary payer type, as well as the number of empty beds and percent occupancy. May be run by unit or entire center. 

Action Summary Report

Provides a summary of changes in resident census information for a specified time frame, including time of event and time of entry into PCC. Also available from clinical dashboard under Admissions view, ADT section.

Admission Record

Form listing demographics, admission dates, diagnoses, contacts, and payer information for a resident. Available also from the Resident Profile screen for an individual resident. Also known as Face Sheet.

Resident List Report

Lists all residents according to status. Can be formatted to include a variety of information, such as birth date, physician, gender, age, religion, marital status, and other user defined information. May include as few or as many of the above categories as needed, as well as other details. 

Admission/Discharge To and From Report

Provides list of residents admitted and/or discharged, including where admitted to or discharged from.

Medicare Days Remaining

Lists all Medicare days remaining for all residents.
Birthday, Religion,  Resident Contacts lists/reports
Separate reports which list each resident with their respective birth date or other specified information. These may be formatted to sort by resident or by the category.

Diagnosis report

Provides a list of all residents with a particular diagnosis or class of diagnoses, or a list of each resident with their specific diagnoses.

Medical Professionals Resident Listing

Provides a list of all the residents and their assigned physician, dentist, etc., or a list of all residents assigned to a particular medical professional.

User-Defined Information Report

Provides a list of residents within specified user-defined information, such as laundry preference, uses facility wheelchair, veteran status and service branch, etc.

Care Plan Item/Task Listing

Report

Provides a list of residents with a specific type of care plan focus or task, such as all with Skin or Fall care plan focus.

Care Plan Library Report

Allows printing of the entire care plan library for reference. 
 

Care Plan Focus Summary Report

Provides a list of the care plan focuses for a resident or all residents.

Care Plan Reviews Due

Provides a list of care plan review dates, either by resident or by date. May also be accessed from the clinical dashboard, in the Care Plans view section.

Missing Entries Report

Provides list of missing information of the type selected, for a selected time frame. For example, residents with no diagnoses, no diagnosis sheet, no allergies, no care plan, no care pan reviews, etc. 

Assessment History Report

Provides list of assessments of selected type, indicating status (completed, in progress, has errors, lock date, accepted date, completion date), as well as key information depending on assessment type. For example, all Braden scales, with status and score, for a time period.

Resident Response Analyzer

Provides a list of residents for which any assessment item(s) is answered in a particular way. For example, those whose MDS is answered under Section N that they prefer music activities, those who have dentures or removable bridges, those who have diabetes and do NOT have special foot care checked, those whose Braden scale indicates they have Very Poor or Probably Inadequate nutrition. Any combination from within an assessment may be selected.

Resident Response Comparison

Provides a comparison of how a particular assessment item was answered over time if more than one assessment of that type has been done. 

RUGs Summary

Lists RUGs groupings, with the number and percentage of residents within that grouping. May be run for Medicare or state if applicable, and with just category or actual Case Mix level. A graph is also viewable and printable.

Case Mix Detail (Assessment Case Mix Report)

Lists residents with the most recent RUG and case mix index score for either Medicare or state case mix. Available from the CMI section of the clinical dashboard.

MDS Billing Calendar

Resident-specific ‘100-day tool’, showing acceptable reference date range for each PPS assessment. Printable from MDS screen for any Medicare A resident MDS.
