Workflow for Section GG
1. 1. Rehab will responsible to screen/evaluate and complete Section GG Admission Assessment and Goals for all Part A residents admitted to the facility on or after October 1, 2016. Rehab Optima has setup this to be completed with the Initial Evaluation. This information will be imported to the PPS 5 day Admission Assessment.
a. For Part A residents not being picked up for therapy programs, therapy will conduct an admission screen and will consult with MDS Nurse to determine coding of usual functioning for Section GG. MDS department will be responsible to complete Admission Assessment and Goals for these residents. As such, PCC should generate 1 User Defined Assessment within the first 3 days of the Medicare stay for all Part A admissions.

All Medicare planned discharges will be discussed at the Facility UR meetings. When a definitive discharge date is selected the Discharge Assessment (UDA) will be opened for the last day of the Medicare stay. This has to be manually selected from the evaluations tab of the resident’s chart.
Therapy will complete the Discharge Section GG Assessment in Rehab Optima which will be auto populated on the MDS.
The MDS Nurse will review all information imported from rehab and complete Section GG on the Discharge MDS. If the resident is not on rehab the MDS nurse will be responsible to complete Section GG on these discharges.
