Quick ADT: Adding a Returning 

(Re-admission) Resident

After a resident is admitted in the system, census changes (admission or re-admissions) can be completed through the Quick ADT application. There are different types of census entries that can be conducted using the Quick ADT link, including a admission or reentry. A admission or reentry includes the recording of a transfer in on return from hospital or return from a leave of absence.

The Quick ADT application allows staff to record the census entry without impacting billing. After completing the Quick ADT, the resident status is updated with an Incomplete census entry. The Quick ADT action requires confirmation by the business office at a later time.

Procedure

  1. Search for the resident's record. Hover over Clinical or Admin to access the menu and select Residents from the People section. Notice Resident status is “NEW”

  1. Click the adt to the left of the resident name in the resident listing OR click the Resident's name to access his/her record and then hover over Clinical or Clinical and select Quick ADT from the Actions Section.

 

  1. In the Quick ADT pop-up, click the Action Code list and select the applicable action-Admission/Reentry.
  2. Set the Effective Date and Time.
  3. Select the Estimated Payer Type and the Primary Payer. The Location field is auto-populated with the resident's room number.
  4. If the resident's room number needs changed, use the Clear link to clear the location. Search is the Location field to select a bed for the resident. In the Location Pick List pop-up, click the link for the appropriate available room.

  1. Select a To/From Type using the list.
  2. Select a To/From Location using the list.
  3. Add any additional available non-required information as desired.
  4. Click Save. Notice Resident status is now Current. 

 

Hints and tips

  • In the case of the readmission/reentry, an MDS schedule is automatically generated for the resident based on the readmission date.
  • When re-admitting a resident, the medical record number can be changed as needed.
  • An icon provides information about the To/From Type and the To/From Location lists.

Business rules

  • The available census entry options are security driven.
  • Incomplete Census items can be viewed on both the Clinical and Admin Dashboards.

 

Quick ADT: Entering a Discharge

After a resident is admitted in the system, census entries admitted can be completed through the Quick ADT application. There are different types of census entries that can by conducted using the Quick ADT link. The Quick ADT application allows staff to record a census entry discharge without impacting billing. Upon completion of the Quick ADT, the resident status is updated with an Incomplete Census entry. The Quick ADT action requires confirmation by the business office at a later time.

Procedure

  1. Search for the Resident's Record. Hover over Clinical to access the Clinical menu. Select Residents from the People section.
  2. Click the ADT link located to the left of the resident name in the resident listing, or click the Resident's Name to access the record. Hover over Clinical, and select Quick ADT from the Actions Section.

  1. In the Quick ADT window that appears, click the Action Code list and select the applicable action: Discharge.
  2. Set the Effective Date and Time.
  3. Select a To/From Type using the list.
  4. Select a To/From Location using the list.
  5. Add additional non-required fields such as Ordered By or Emergency Transfer, Reason for Transfer, Unplanned/Planned, and Outcome of Transfer.
  6. Click Save.

Hints and tips

  • Resident status is now Discharge.
  • An  icon provides information about the To/From Type and the To/From Location lists.

Business rules

  • Incomplete Census items can be viewed on both the Clinical and Admin Dashboards.

Quick ADT: Entering a Discharge 

Due to Death

After a resident is admitted in the system, census entries (discharge due to death or deceased) can be completed through the Quick ADT application. There are different types of census entries that can be conducted using the Quick ADT link. The Quick ADT application allows staff to record a census entry without impacting billing. Upon completion of the Quick ADT, the resident status is updated with an Incomplete census entry. The Quick ADT action requires confirmation by the business office at a later time.

Procedure

  1. Search for the resident's record - hover over Clinical or Admin to access the menu and select Residents from the People.
  2. Click the adt link located to the left of the resident name in the resident listing OR click the Resident's name to access his/her record and then hover over Clinical and select Quick ADT from the Actions.
  3. In the Quick ADT pop-up, click Action Code and select the applicable action:
    • Set the Effective Date and Time.
    • Complete additional non-required fields as desired.
  4. Click Save.

  • Incomplete Census items may be viewed on both the Clinical and Admin Dashboards.

Quick ADT Entering a Room 

Change

After a resident is admitted in the system, census entries (room changes) can be completed through the Quick ADT application. A roomchange entails recording an internal transfer when a resident is moved from one bed to another bed within the facility.

Different types of census entries can be conducted using the Quick ADT link. The Quick ADT application allows staff to record a census entry (room change) without impacting billing. Upon completion of the Quick ADT, the resident status is updated with an Incomplete census entry. The Quick ADT action requires confirmation by the business office at a later time.

Procedure

  1. Search for the resident's record. Hover over Clinical to access the Clinical menu and select Residents from the People section.
  2. Click the adt link to the left of the resident name in the resident listing OR click the Resident's name to access the record and then hover over Clinical and select Quick ADT from the Actions section.
  3. In the Quick ADT window, click the Action Code list and select room Change.
  4. Set the Effective Date and Time. The Location field may be auto-populated with the resident's room number.
  5. To change the resident's room number, use the Clear link to clear the location. Click the magnifying glass at the end of the Location field to select a bed for the resident. In the Location Pick List window, click the link for the appropriate available room.
  6. Complete the fields as required.
  7. Click Save.

  • Incomplete Census items can be viewed on both the Clinical and Admin Dashboards.

Quick ADT: Entering a Transfer 

Out

After a resident is admitted in the system, census entries (Transfer Out) can be completed through the Quick ADT application. Transfer out may apply to a transfer to the hospital or a resident leaving on a leave of absence. Different types of census entries can be conducted using the Quick ADT link. The Quick ADT application allows staff to record a census entry without impacting billing. Upon completion of the Quick ADT, the resident status is updated with an Incomplete census entry. The Quick ADT action requires confirmation by the business office at a later time.

Procedure

  1. Search for the resident's record - hover over Clinical to access the Clinical menu and select Residents from the People section.
  2. Click adt located to the left of the resident name in the resident listing OR Click the resident's name to access the record, hover over Clinical, and select Quick ADT from Actions.
  3. In the Quick ADT pop-up, click the Action Code list and select the applicable action: Transfer Out.
  4. Set the Effective Date and Time.
  5. Select a To/From Type using the list.
  6. Select a To/From Location using the list.
  7. Populate additional non-required fields such as Ordered By or Emergency Transfer, Reason for Transfer, Unplanned/Planned, and Outcome of Transfer.
  8. Click Save.
